Coordinated care around the individual
We work in partnership with professionals involved in an individual’s care and support, including local authorities, Community Mental Health Teams, GPs, NHS services, housing providers, social workers, mental health professionals and other health and social care professionals.
Our referral and review approach is designed to keep the individual’s needs, preferences, safety and goals at the centre while supporting clear communication between the relevant parties.
